Uniquely Yours Maternal Health Bridge Program
Closing the Gap. Continuing the Care.
Because your healthcare journey doesn't end after childbirth.
Pregnancy and childbirth bring significant changes to a woman's body, but the need for medical care doesn't end when the baby arrives or after the traditional six-week postpartum visit.
At Uniquely Yours Women's Wellness Clinic, we believe the first year after childbirth deserves continued attention, individualized support, and a clear pathway to long-term wellness.
Our Maternal Health Bridge Program is a specialized postpartum-to-primary-care transition program designed to help women navigate their health beyond childbirth. Through comprehensive medical assessments, ongoing follow-up, preventive care, and coordinated healthcare services, we help bridge the gap between obstetric care and lifelong primary care.
Whether you're recovering from an uncomplicated pregnancy, managing a pregnancy-related medical condition, experiencing emotional changes, or simply unsure where to turn for your own healthcare after delivery, we're here to support you.
More Than a Six-Week Checkup
For many women, the traditional postpartum appointment marks the end of routine obstetric care. However, postpartum recovery and health needs often continue well beyond that visit.
Pregnancy-related conditions such as high blood pressure, gestational diabetes, and perinatal mental health concerns can require continued monitoring and treatment. Other concerns, including thyroid changes, fatigue, sleep difficulties, and chronic health conditions, may also emerge or continue during the postpartum period.
Our program focuses on ensuring these concerns are recognized, addressed, and appropriately managed as women transition into ongoing primary care.
Your postpartum recovery matters. Your long-term health matters, too.
Who Is the Maternal Health Bridge Program For?
Our program is designed for women during the first 12 months after childbirth, including those who:
Are transitioning from obstetric care and need continued medical follow-up.
Do not currently have an established primary care provider.
Experienced gestational diabetes, preeclampsia, gestational hypertension, or other pregnancy-related complications.
Are navigating postpartum depression, anxiety, or emotional adjustment.
Need continued monitoring or management of existing health conditions.
Have concerns about postpartum recovery, fatigue, thyroid health, or overall wellness.
Want a more coordinated approach to preventive care and long-term health.
You do not need to have experienced pregnancy complications to benefit from the program.

Your Journey Through the Bridge
Step 1 — Your Initial Health Assessment
We begin by reviewing your pregnancy, delivery, medical history, and current health concerns to identify your individual needs and establish a personalized care plan.
Step 2 — Fourth Trimester Follow-Up
During the early postpartum months, we focus on recovery, physical and emotional wellness, and follow-up for pregnancy-related conditions.
Step 3 — Transition Into Primary Care
As your postpartum needs evolve, we focus on preventive healthcare, ongoing medical management, and establishing a long-term primary care relationship.
Step 4 — Continued Wellness
Follow-up visits throughout the remainder of your first postpartum year help us evaluate your progress, address new concerns, and support your health goals.
Step 5 — Lifelong Healthcare
Our goal is to help you move beyond postpartum recovery with an established healthcare plan and continued access to appropriate primary care.
Visit timing and services are individualized based on your medical needs. Urgent concerns and certain pregnancy-related conditions may require earlier assessment.
You Don't Have to Navigate Postpartum Healthcare Alone
Whether you're six weeks postpartum, six months into motherhood, or approaching your baby's first birthday, your health deserves continued attention.
At Uniquely Yours Women's Wellness Clinic, we're committed to helping you move confidently from postpartum recovery into long-term wellness.
Beyond Birth. Beyond Recovery. Wellness for Life.
